Most courses are delivered inside host institutions for their own staff. That is the model: hospitals certify their own trainers and run CSU-ALS continuously in-house.

Become a Training Center. Hospitals bring ECSR in-house through CARE's formal training-center pathway. Accredited Centers train their own staff with CARE-certified trainers under a formal agreement, with biennial audits beginning at the end of the first year and no fees to maintain status. Centers of Excellence, after a successful first audit, may open courses to outside staff, receive a plaque for display in the unit, become eligible for national teaching opportunities and CARE research, and maintain 90% of staff trained at all times. Accredited Centers may upgrade without additional fees.
